چکیده

Most patients with cardiac arrhythmias require treatment with antiarrhythmic drugs, although recent advances in surgical and electrical techniques are promising. The ideal antiarrhythmic compound should have: (1) A wide range of therapeutic activity in both atrial and ventricular arrhythmias. (2) Parenteral and oral formulations to permit simple dosage schedules. (3) Pharmacokinetic properties that permit long term prophylactic treatment. (4) Lack of serious cardiac depressant activity or non-cardiac side effects. Although the ideal agent has not yet been found, many drugs are now under investigation. Here we review the drugs in current clinical use and refer to other compounds at present

Clinical effectiveness of antiarrhythmic treatment after electrical cardioversion in patients without structural heart disease.

INTRODUCTION AND OBJECTIVES The aim was to investigate the clinical effectiveness of using antiarrhythmic drugs after electrical cardioversion in patients without structural heart disease, including their effect on the maintenance of sinus rhythm. METHODS In total, 528 patients with persistent atrial fibrillation but without significant structural heart disease who underwent successful electr...

Evaluation of Teaching Clinical Pharmacology of Antiarrhythmic Drugs to First-Year MBBS Students Through Worksheet-Based Small-Group Discussion

Cardiac arrhythmias are a major cause of morbidity and mortality across the world. Learning the science behind the use of antiarrhythmic drugs is essential for all medical graduates. However, many antiarrhythmic drugs are available, and most of them have complex pharmacodynamic and pharmacokinetic profiles. We tried to improvise our teaching by conducting interactive, worksheet-based, small-gro...

Pharmacological effects of antiarrhythmic drugs. Review and update.

Most antiarrhythmic drugs are potent compounds with a relatively narrow therapeutic index. When prescribed judiciously, they can have a key role in enhancing or prolonging the lives of patients with most common arrhythmias. But when misprescribed, through selection of an inappropriate drug or dosage regimen, the end result may range from inadequate control of the arrhythmia to a proarrhythmic e...

Radiofrequency catheter ablation of accessory pathways. Outcome and use of antiarrhythmic drugs during follow-up.

AIMS The purpose of this study was to assess the acute and long-term success of accessory pathway ablation in a single large-volume centre, concentrating on long-term recurrences and the clinical use of antiarrhythmic drugs. METHODS AND RESULTS A total of 519 consecutive patients (mean age 40+/-14 years) underwent radiofrequency ablation of manifest or concealed accessory pathways. The patien...

Interactions between antiarrhythmic drugs and food.

OBJECTIVE A drug interaction is defined as any alteration, pharmacokinetics and/or pharmacodynamics, produced by different substances, other drug treatments, dietary factors and habits such as drinking and smoking. These interactions can affect the antiarrhythmic drugs, altering their therapeutic efficacy and adverse effects. The aim of this study was to conduct a review of available data about...

Experimental evidence for proarrhythmic mechanisms of antiarrhythmic drugs.

The major limitation to antiarrhythmic drug therapy is the risk of arrhythmia promotion, or 'proarrhythmia.' This complication may be lethal, and greatly restricts the value of antiarrhythmic agents, particularly for arrhythmias without an intrinsic mortality risk, such as atrial fibrillation. In order for improved antiarrhythmic drug therapy to be developed, it is essential to understand the f...
